Description The World Health Organization (WHO) is actively assisting nations in developing their health systems to achieve and sustain universal health coverage, while also monitoring the progress.

Describe the policy in each country related to the identified healthcare issue United States: Fee-for-service In this policy, healthcare providers receive fees for each service they provide to patients. Regardless of the quality of care, physicians are compensated based on the volume and quantity of services rendered (Reid, Damberg, & Friedberg, 2019).

China: China National Essential Medicines Policy (NEMP) The policy was established to ensure the consistent distribution of medicines and other essential healthcare supplies across the country at a low cost and without profit (Mao, Huang, & Chen, 2019).

What are the strengths of this policy? United States:

  • Patients receive excellent service, and providers can suggest improvement strategies for better outcomes.
  • Clinicians can charge a fair price for a service and be flexible enough to provide precise help to different patients.

China:

  • The policy increases the number of people who use medical services across the country.
  • It regulates drug costs and ensures non-profit delivery of pharmaceuticals to healthcare facilities, cutting both outpatient and inpatient costs at the grassroots level.

What are the weaknesses of this policy? United States:

  • The fee-for-service policy results in increased inpatient and outpatient costs. The U.S. healthcare system is more costly than in other countries.

China:

  • Adequate access to prescription drugs may lead to misuse and overuse. Overuse of injections is frequent in Chinese healthcare facilities, particularly in primary care settings. This not only wastes medical resources but also increases the risk of infections by viruses such as hepatitis C and AIDS (Xiang, Yang, Wang, Ye, & Zhang, 2012).

In a fee-for-service environment, social needs are economically unsustainable. While fee-for-service payment incentivizes clinicians to provide more services to consumers, value-based care focuses on keeping patients out of doctors’ offices and pushes providers to address societal issues that worsen or cause serious health problems.

For example, Ikegami (2015) found that physicians tend to deliver more and unnecessary services to maximize their income.

Also, the fee-for-service policy has led to excess out-of-pocket spending for individuals with chronic conditions. Fong (2019) pointed out that certain chronic diseases exert a greater financial toll on the elderly, especially with the fee-for-service program.

Cost-related non-adherence (CRN) is common among people with low income, and it can have serious health effects on populations with chronic infections.
