Project Introduction
As emphasized by Birmingham, Buffum, Blegen, & Lyndon (2015), the establishment of efficient and effective communication frameworks within healthcare organizations is crucial for advancing care delivery and averting undesirable events such as medical errors, communication breakdowns, and failure to rescue. In alignment with this imperative, this paper introduces my capstone project, focusing on the implementation of SBAR to enhance communication. Identification of Problem
The identified problem stems from communication failures experienced while serving as a charge nurse in the urgent care unit at Kaiser Permanente, a standalone clinic. Challenges arise in ensuring seamless patient transfers to nearby healthcare facilities due to inadequate reporting and communication with receiving nurses and other healthcare providers. In instances where a patient requires a higher level of care or hospitalization, we coordinate with medics or emergency medical technicians to transport patients to the Los Angeles Sunset facility, where reporting to the receiving care provider is mandated.NR 660 Week 2: Project
The difficulty lies in reporting off to external personnel, as we often lack crucial information about our patient’s status, whether a handoff has occurred, and if so, whether it was conducted appropriately. This lack of communication results in delayed care provision, subsequently impacting health outcomes.
It is evident that our healthcare organization is not alone in facing these challenges, and there are numerous other organizations that could benefit significantly from this project. Addressing the rise in medical errors, communication breakdowns, and failures to rescue is imperative (Randmaa, Mårtensson, Swenne, & Engström, 2014). To guide the project research, the developed PICO question is: “In healthcare settings (P), does the use of the SBAR communication tool (I) compared to informal communication channels (C) lead to improved communication during patient handoffs (O)?”
By addressing this question, the project aims to contribute valuable insights and evidence to the broader healthcare community, facilitating the development and implementation of effective communication strategies during patient handoffs. The anticipated benefits extend beyond our organization, fostering improvements in patient care and safety across diverse healthcare settings.
