To formulate a precise clinical question that encapsulates the essence of the project, the following query is proposed:
“In elderly patients suffering from congestive heart failure (P), does the implementation of a comprehensive discharge plan (I) compared to standard care (C) contribute to a reduction in hospital readmissions (O) within a 30-day timeframe (T)?”
